The ATM Midwifery Training Program is required by Texas law to retain records on preceptors and course instructors. Preceptors must meet minimum experience requirements of either 3 years experience since licensure/certification, or 50 births since licensure/certification. A complete list of requirements is available from the Course Coordinator.
Please fill in the applicable portions of the application forms , sign, and date. Submit with the appropriate items to the ATM office:
- Filled in and signed Preceptor Application form
- Signed Preceptor Agreement (Included with application forms.)
- Copy of current license
- Copy of your Resume™ or CV if you are not a CPM
- Proof of CPM ( copy of card or certificate ) if applicable
- Non-CPM out-of-state licensed midwives must submit copies of CPR and Neonatal Resuscitation Certification cards
- Request a copy of the Preceptor Handbook from the Course Coordinator
