A list of all current midwives able to train a new midwife
Application Instructions
The ATM Midwifery Training Program is required by Texas law to retain records on preceptors and course instructors. Please fill in the applicable portions of the application forms , sign, and date. Submit the appropriate items to the ATM office:
- Filled in and signed Preceptor and/or Instructor Application form
- Signed Preceptor Agreement contract (You may print out this online form and use it to sign and mail in.)
- Copy of current documentation/license
- Copy of your ICA/Resume™
- Proof of CPM ( copy of card or certificate ) if applicable

You must maintain ATM Associate Membership status to serve as preceptor for ATM students. CPR/NNR must be sent in yearly with membership renewal.
